I haven't booked or gone yet, but I began my planning in earnest when a friend told me about a vacation he took last year. My fiance and I were already planning to go to Italy, but didn't know if we could get a good deal. Alitalia has a travel plan division called www.italiatour.com. They have just plane, plane and hotel &/or car &/or train, etc. They can customize tours for you, with or without a guide. So I would recommend generalizing this experience, to say that you should check national airlines for the country you want to visit, and see if they have plans. They tend to be high quality, and have connections, if not direct support from the govenment, to give you the best experience in that country for the best money.
